Voicepack issues after latest update

This is the error I'm getting:

9:59:27.171 Error initializing sound - Stream failure : Unable to acquire output device for audio playback.

Any suggestions?

VA settings -> Audio Tab.

If you have VA set to select a specific audio device on start up (or to change to it), I would suggest reselecting it on the dropdown list. If the device is listed twice, choose the *OTHER* one. Chances are that windows updated the audio device list, and VA (because you are loading a specific setup on startup) is looking at the *OLD* Device ID
